Working monolithically has many advantages – the risk of chipping is significantly reduced compared to fully veneered restorations. The space that actually has to be planned for the veneer is eliminated, allowing work to be carried out in a more minimally invasive manner. Due to the production in the digital workflow, such supplies can be manufactured more economically than in the analogue way. However, for monolithic restorations to be successful, the CAD/CAM material must meet several basic requirements. In the following interview, dentist and dental technician Dr. Julio Gomez Paris (Rosario, Argentina) reports on his experiences with the zirconia system VITA YZ and why he is particularly enthusiastic about the super-translucent and polychromatic material variant VITA YZ ST Multicolour in the production of monolithic restorations, and demonstrates this using a bridge restoration in the anterior mandible.

You are both a dentist and a dental technician. What is your experience with the zirconia VITA YZ in terms of precision, edge stability and fit?

As a dentist and dental technician, I always experience the complete supply workflow and have a complete overview of the precision, from the preparation and the intraoral scan, to the milling strategy, sintering, fitting and integration. I have already tried a number of zirconias, and from my experience I can say that VITA YZ is my absolute favourite zirconia. The sintering parameters are correct, the precision and fit are excellent. Edge areas can be milled very finely and have absolutely stable edges. In short, VITA YZ offers me what I expect from a zirconia.

Depending on the indication, VITA YZ is available in four different translucency levels with different strengths. Which zirconia variants do you use and for what purpose?

I use the extra translucent VITA YZ XT for monolithic anterior crowns on undyed tooth stumps, and the super translucent VITA YZ ST on discolored stumps or abutments for crowns and bridges with dentogingival involvement. I use the highly translucent VITA YZ HT for screw-retained implants in edentulous jaws. Here and with very dark or metallic stumps, I work in conjunction with the universal veneering ceramic VITA LUMEX AC to reproduce the gingival and dental facets and, if necessary, as a support in masking them.

Is the zirconia VITA YZ ST Multicolor a material for all fixed indications? For which clinical situations do you specifically use the material and why?

Yes, VITA YZ ST is a true versatile materials that covers most indication areas. Especially as a multicolor variant, this zirconia, with its integrated chroma gradient, is perfect for monolithic restorations. The blank already possesses great aesthetics; therefore, in many cases, it does not require individualisation with a veneering ceramic. Monolithic crowns and bridges are the prime applications for this material, but individualisation or characterisation is also always possible. I also like to use the super translucent zirconia monolithically on implants. In the case of dentogingival reconstructions, purely gingival customisation is often sufficient.

Why did you decide on VITA YZ ST Multicolor in the present case of mandibular incisors with previous periodontal damage?

For the bridge in the anterior mandible, I have consistently opted for a monolithic solution because I wanted to be as minimally invasive as possible. My top priority was to preserve the vitality of the delicate teeth through careful preparation. As a basic requirement, VITA YZ ST Multicolor offers me a minimum layer thickness, as well as the strength required for a bridge construction. At the same time, the polychromatic material also makes it possible to achieve excellent aesthetics monolithically, so that I don’t have to plan any space for the veneer.

What is the advantage of the integrated chroma gradient in combination with the basic shade fidelity to the VITA shade standards?

It is definitely an advantage if the specific basic tooth shade can be found precisely in the blank. In my clinical experience, the zirconia system VITA YZ has proven to be absolutely true to shade. The VITA SYSTEM 3D-MASTER also completely covers the range of tooth shades, so that I can find the right shade for every patient. For me, this shade fidelity is a basic requirement so that I can successfully work monolithically with a material and achieve aesthetic results, especially in the anterior tooth region. In general, it saves a lot of time if I don’t have to first set the right shade with corrective firing.
